The manufacturer William Cook Europe ApS informs about the recall of some tracheostomy-sets. The sets contain Shiley FlexTra Tracheostomy Tubes which are originally recalled by Tyco Healthcare. It is possible that the outer cannula separates from the head/flange assembly. The outer cannula could potenially travel into the patient's airway and significantly interfere with ventilation.
