The company Invacare Aquatec has issued a field safety notice about a retrofit of the wheelchair REA Azalea Base. The retrofit applies to all REA Azalea Base produced between March 2008 and May 2009. There is the risk that the bracket at the rear of the seat rails below the pivot point can fracture if the wheelchair is exposed to excessive load, possibly resulting in a patient fall.
