Varian Medical Systems has issued a Field Safety Notice concerning a corrective action of the AC 250 ISC Varian Proton Therapy Delivery System. If the number of fractions which are being requested by the treatment planning system is not verified, it is possible that the entire dosis will be delivered in a single fraction. The TBCOO1 software will be modified to implement a check with a password authorization when a single fraction treatment is ordered instead of a fractionated plan.
Only one customer in Germany is affected by this recall.
