Varian medical systems has issued a Field Safety Notice concerning a problem with guiding Tube to disconnect from Cervical Sleeve. The guide tube is connected to an implanted cervical sleeve, and secured by gripping jaws which collapse during disassembly. Due to a manufacturing error in lot L33, the gripping jaws do not disconnect easily from the cervical sleeves.
