Accuray International Sàrl has issued a Field Safety Notice concerning a corrective action of the CyberKnife Iris Variable Aperture Collimator Option (excluding M6 Series). A defect on the master-side connector block of the pneumatic tool changer may introduce a tilt in the collimator housings, resulting in a potential shift to the central axis of the radiation beam.
